UWEEI Hardship Grant Form

 

We are pleased to inform you that UWEEI will be launching a hardship grant to assist union members who are suffering from financial hardship. Starting in December 2005, this is an on-going assistance scheme and members who fall within the criteria are eligible to apply throughout the year.

 

The scheme covers four aspects namely: hospitalization/ long-term illness, death of member, natural calamity and others. “Others” refer to any other situations, which has resulted in the member falling into financial difficulties. UWEEI will be providing a one-time grant in cash of up to $1000 to members to help defray their financial burden. As a start, $50,000 will be set aside from general fund.

 

Union members who fulfill the following criteria below can apply for the assistance:

  • Member must hold membership with the union for at least 6 months
  • Eligible member shall only receive one hardship grant from the Union within the period of 3 years

 They must also satisfy the conditions (listed below) to which they apply for the grant.

 

Coverage

Criteria

a. Hospitalization/ Long-term illness-

 

  • Member has exhausted all his/her    hospitalization leave and 
  • He/she is suffering from financial hardship

 

b. Death of member-

The scheme assists the children of the member who has passed away.  The grant will be given to the family for the purchase of their children’s school textbooks, uniform and stationary.

 

  • Spouse is not working or
  • Spouse earns a low income

 

c. Natural calamity

 

  • Member suffers from natural calamities such as fire and tsunami

 

d. Others

 

  • Members who suffer from other forms of financial hardship are also eligible to apply for the grant.

 

 

A copy of the application form is enclosed herewith for your member’s use.  Please disseminate the information to your members and make photocopies of the form for those members who are interested to apply. 

 

Please note that only one application per family needs to be submitted.

 

All completed forms are to be returned to UWEEI HQ together with the following applicable documents:

 

v      Member and his/her spouse’s latest payslips

v      Death certificate (if member or spouse is deceased)

v      Divorced certificate or divorce petition (if member is a divorcee)

v      Medical report, Medical Certificates or Doctor’s letter (if member suffers from long- term illness)

v      Other relevant documents to substantiate claims
